As a Registered Nurse in Home Care, I promote and maintain the
health of individuals within the community by providing
personalized care, conducting assessments, and developing care
plans. I help patients and families navigate their healthcare needs
while delivering comprehensive nursing services and support. My
role includes coordinating home care delivery, maintaining clinical
documentation, and promoting self-help strategies for emotional and
physical challenges.

counseling and appropriate preventive and rehabilitative services.
-Responsible for assessment of health needs and contributing to the
plan for care for individuals and families. -ElderONEAn affiliate
of Rochester Regional Health, a Program of All-Inclusive Care for
the Elderly (PACE). Servicing Monroe, Ontario and Wayne counties,
ElderONE is an innovative alternative to long-term nursing home
placement. - - As the area's only PACE program, ElderONE represents
the future of senior health care. This long-term care program
provides the entire continuum of medical care and support services
to seniors 55 and older -with chronic care needs, allowing them to
maintain their independence in the comfort of their home without

ShiftATTRIBUTES
- AAS degree in Nursing required.
- License and current registration to practice as Registered
Professional Nurse in New York State
- Current NYS Driver's licenseRESPONSIBILITIES
- Patient Care. Conduct initial assessments; establish plan of
care and provide prescribed treatments and demonstrated nursing
abilities; coordinate the delivery of 24-hour care;
- Communication. Counsel and guide individuals and families
towards self-help in recognition and solution of physical,
emotional and environmental problems
- Evaluation & Documentation. Conduct reassessment of each
patient on a regular basis; maintain complete and accurate clinical
documentation; supervise and serve as a resource to all home care
staffEDUCATION:AS: Nursing (Required)LICENSES / CERTIFICATIONS:
-BLS - Basic Life Support - American Heart Association (AHA), RN -
Registered Nurse - New York State Education Department
(NYSED)PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50
pounds of force occasionally, and/or 10 to 25 pounds of force
frequently, and/or greater than negligible up to 10 pounds of force
constantly to move objects; Requires frequent walking, standing or
squatting.For disease specific care programs refer to the program
specific requirements of the department for further specifications
on experience and educational expectations, including continuing
education requirements. Any physical requirements reported by a
prospective employee and/or employee's physician or delegate will
be considered for accommodations. PAY RANGE:$73,528.00 -
$98,072.00CITY:RochesterPOSTAL CODE:14617The listed base pay range
is a good faith representation of current potential base pay for a
successful full time applicant. It may be modified in the future
and eligible for additional pay components. Pay is determined by
factors including experience, relevant qualifications, specialty,
internal equity, location, and contracts.Rochester Regional Health
