- Period of peformance is 12/1/15- 5/30/16
- Option to extend available for 2 three month extensions
- RFQ close date is November 11th at 10am

- Any state license accepted
- 12-14 patients per day
- M-F 8a-4:30p
- BC in IM or FP
- DEA
